RICE Kitchen catering design

RICE Kitchen needed a comprehensive form to collect Catering orders from their website.

I used Formidable Forms software to generate the code and formatted it in two pages to capture all the important data, the mobile version stacks for a smaller viewport.  I updated the header image so it was more on brand and reflected catering dishes that are present in the form. I also added in a few catering photos to the bottom with the SEO text.
